During your initial evaluation, we will work with you and your physician to create a treatment plan that will meet all of your goals.

For that first visit, please remember to do the following:

•  Arrive 10-15 minutes prior to your appointment to fill out paperwork (except 1 p.m. appointments).
• Bring your insurance ID cards (both your primary cards and your secondary or supplemental cards).
• Bring your driver's license or TX ID card.
• Bring your referral from your physician.
• Bring a list of all medications you are currently taking.
• Bring any written reports from an MRI, CAT Scan, or x-ray if you have them.
• If workers' compensation, bring all work, insurance, and adjuster information.
• If you have Home Health, bring the agency information/phone number.
• Wear comfortable clothing that you can move easily in.
• Be prepared to spend about 90 minutes to 2 hours here for your first visit.
